(SPOT.ph) If you’re living with a disability—whether visible or not—you could be entitled to some perks that can make life a little easier. From discounted medicine to priority lanes to tax exemptions, having a person-with-disability (PWD) ID provides access to a wide range of support and services. Getting a PWD ID is free and it is recognized as a valid government ID, according to the National Council on Disability Affairs (NCDA).
So where to start if you're looking to apply? Here's a step-by-step guide to getting your PWD ID.
Who can apply for a PWD ID
As of September 2024, the Department of Health (DOH) has recorded over two million PWDs across the country, with almost 320,000 living in Metro Manila.
Note that not every health issue qualifies you for a PWD ID. Under Republic Act No. 10754, individuals must have "long-term physical, mental, intellectual, or sensory impairments" that limit their ability to function or participate fully in society. Here’s a breakdown of the conditions that make you eligible:
- Physical disabilities: Loss of limb function, mobility issues, or chronic illness-related disabilities.
- Visual or hearing impairments: Partial or full blindness or deafness.
- Mental or intellectual disabilities: Conditions like autism, schizophrenia, or bipolar disorder.
- Speech and language disabilities
- Cancer
- Rare disabilities
Even cancer survivors are now considered eligible under the National Integrated Cancer Control Act, which recognizes the long-term impact of the disease.
The PWD ID is valid for five years. However, if you need to renew, replace, or get a damaged card reissued, a minimal fee may be charged. To renew, you must provide the same set of documents you submitted for your initial application.

How to apply for a PWD ID
The application process for a PWD ID in the Philippines can vary depending on your location, as different local government units (LGUs) may have specific procedures in place. However, the general requirements are more or less consistent nationwide.
Of course, the applicant should be a resident of the city or municipality where the PWD ID application is being processed.
Prepare the required documents
- Application form: Print and fill out the application form or get one from your local Persons with Disability Affairs Office (PDAO), City/Municipal Social Welfare Office (CWSDO/MSWDO).
- Medical certificate: This should come from a licensed physician and must detail your disability. For mental health conditions, you’ll need a certification from a psychiatrist or psychologist.
- Two ID photos: 1x1 or 2x2 photos with your name and signature/thumbprint on the back.
- Valid government ID: A photocopy of any government-issued ID or a barangay clearance to prove your residency.
There isn’t a comprehensive, nationwide directory yet that compiles all the CSWDO/MSWDO or PDAO locations so this information often needs to be gathered on your own.
To find the nearest CSWDO/MSWDO or PDAO, you can visit your LGU's official website. Many LGUs list the contact information and addresses of government offices, making it easier to locate them. Alternatively, most LGUs also have public service hotlines where you can directly inquire about these offices and the services they provide, including processing PWD ID applications.
Submit the application
Walk-in application
Head to the local PWD office in your area with your completed form and documents. Most PWD IDs are processed at LGUs. If necessary, you may be asked to undergo an assessment by a medical professional to verify your disability.
Processing times vary by locality, but the ID can sometimes be issued within the same day (yes, that fast!). In other locations, it might take a few days to a week. You can also verify the status of your PWD ID on their official website.
After approval, the PWD ID is issued immediately, along with a purchase booklet, to get a 20% discount on medicines and a 5% discount on basic necessities and commodities.
Online application
A few cities like Quezon City and Pasig City offer online PWD registration to make the process more convenient.
However, some of the online processes are not entirely paperless. After submitting your online application, you will still need to visit your local PDAO to verify information, complete the process, and claim your PWD ID in person.
Quezon City
- Register at QC E-Services using personal email.
- Confirm and validate E-Services account.
- Go back to QC E-Services and log in using the registered email account and choose QCitizen ID eApplication icon.
- Complete and confirm details of personal information and make sure to tick “Yes” for the Persons With Disability Sector.
- Upload all requirements. Upon submission, the virtual ID will show. If the information is correct, agree and submit.
- After the process, an email will be sent to confirm the application.
- Monitor ID application status in the E-Services account.
Pasig City
Fill out the application form and send it to pcpdaoonlineapplication@gmail.com along with the other required documents.
In the email, indicate whether your application is that of a New Applicant, a Renewal, Transfer, or Conversion to PWD ID. Make sure, too, that your photo is clear in the attachment.
Application by proxy
For those unable to physically visit the office, a guardian or caregiver can apply on your behalf. You’ll need to provide a notarized authorization letter signed by the PWD, copies of the PWD’s medical documents and ID, along with the authorized representative's ID.
In some cases, LGUs offer home visits for bedridden applicants so it’s worth inquiring from your local PDAO if this service is available in your location.
